PPHI Sindh is a non-profit organization that operates under section 42 of The Companies Act 2017. It is recognized as an NPO (Not for Profit Organization) by the Pakistan Centre of Philanthropy, a certified government body that acknowledges such organizations. The Board of Directors, which is composed of seven experienced members, governs it. Additionally, it has ex-officio members of the board, who are Secretaries to the Government of Sindh in Health, Finance, Planning & Development, and Population Welfare. PPHI Sindh is registered with the Economic Affairs Division of the Government of Pakistan, enabling it to receive foreign contributions as a local NGO.

The primary aim of establishing PPHI was to improve healthcare services in rural areas of Sindh. Its immediate objective was to ensure that the health facilities entrusted to PPHI were fully operational and accessible to all. To achieve this, a capable management team was appointed and took swift actions. As a result, the health facilities saw regular doctor and staff attendance, and a constant supply of medical resources was maintained.
PPHI manages 1,174 health facilities in 22 districts of rural Sindh, making it the primary healthcare provider in the region. These facilities are equipped with labor rooms and ultrasound machines, and staffed with female doctors and PPHI Online Job Portal trained midwives, even in remote areas. All patients receive free medicines and lab tests.
In 2017, the People's Primary Healthcare Initiative (PPHI) in Sindh had job openings for more than 21 positions in different cities and categories.
